Preparing For Your Visit

What I need for my sleep surgery appointment:
- Prior to your visit you will need a copy of a recent diagnostic sleep study, preferably one done within the last 3 years. The full report should be sent to our office
- A referral from your primary care or sleep medicine provider.
- Notes from your provider forwarded to the office.
For your appointment:
- Complete the sleep surgery intake form prior to your visit.
- Bring your insurance card, a list of your medications, and your physicians’ contact information.
- Review videos about sleep apnea and all the treatment options so you can be prepared for your visit with questions.
At your visit with your provider:
- You will discuss your sleep symptoms and goals of treatment.
- You will have a complete head and neck exam, including a flexible fiberoptic laryngoscopy to examine your nasal cavity and upper throat.
- A conversation about options for therapy and next steps in sleep apnea treatment.
