In order to ensure a seamless patient experience, we may ask that you or your referring provider send us additional information by fax or mail. As a specialty department, we may need to perform diagnostic procedures, testing, and/or therapy during the course of a visit in the best interest of your care. These services may be billed separately from the office visit with our providers. We encourage our patients to call their insurance company before their appointment to determine if specific services are covered under their plan, and how much they may be required to pay out of pocket based on the individual's co-insurance/deductible plan.
- A formal referral, indicating the patient’s diagnosis and clinical reason for referral
- Insurance preauthorization for the visit if applicable to the patient’s insurance plan
- Patient’s demographic information (full name, address, date of birth, contact phone numbers), including a copy of insurance card(s)
- The patient’s previous clinical notes pertinent to the reason for referral, including consult notes, prior lab and test results (such as audiograms), MRI/CT (discs in dicom format), barium swallow or sleep studies, and pathology slides
Clinical Services We Offer
General Otolaryngology - Head and Neck Surgery particular specializes in all aspects of the head and neck. We are experts in endoscopic sinus surgery, rhinology, laryngology, dysphagia, salivary gland surgery, snoring and obstructive sleep apnea.
At the Airway, Voice, and Swallowing Center, we provide diagnosis and appropriate treatment to a wide range of voice and speech complaints, swallowing complaints, and other disorders of the upper airway, including the larynx and the trachea.
We provide extensive service and comprehensive care for patients with benign and malignant tumors of the head and neck areas that include the lips, oral cavity, tongue, sinus, voicebox, throat, jawbone, above and behind the nose.
Our Facial Plastic and Reconstructive Surgery team performs functional and cosmetic rhinoplasty as well as rejuvenation and cosmetic surgery of the face, neck, cheek, brow, and eyelids. Our team of providers offer quality prescriptive care and evidence-based surgical and non-surgical options to meet a wide range of patient needs.
We provide expert care for hearing, balance, facial nerve disorders, inner ear and related skull base brain tumors, and diagnosis and treatment of complex vestibular problems. We also offer a variety of services in audiology, hearing aid, cochlear implant, skull base surgery, balance and falls.
Pediatric Otolaryngology - Head and Neck Surgery offers comprehensive care for children with disorders of the ear, nose, throat, head and neck. We are a specialized team of the best physicians that individualize a care plan for your child, providing our services to communities at one of our Bay Area locations.
The Sinus Center specializes in minimally invasive endoscopic approaches for treatment of disorders of the nose, sinuses and other related structures. This includes treatment of sinus infections, mucoceles, growths and tumors of the nose and sinuses, and nasal obstruction.
